For Referring Providers
Fax referrals to 360-282-1043. We accept referrals for all headache and migraine disorders. We’ll contact the patient directly to schedule and will send visit notes back to your office promptly.

We’re an insurance-based clinic. If you have questions about coverage, copays/coinsurance, or prior authorization, text us and we’ll help you figure out next steps.
