For Referring Providers
Fax referrals to 360-282-1043. We accept referrals for all headache and migraine disorders. We’ll contact the patient directly to schedule and will send visit notes back to your office promptly.
Transparent Cash Pricing
New patient $349 ($249 for Sora Members), follow-up $149 ($99 for Sora Members), $49/month membership. HSA/FSA accepted. CrowdHealth and health share compatible — we provide superbills and handle medication prior authorizations through your pharmacy benefit.
