Insurance

Coverage, copays, and authorizations

We accept Medicare, Medicaid (including Apple Health), TRICARE, and major commercial insurance — including plans such as Blue Cross Blue Shield (e.g. Regence), Premera, Aetna, United Healthcare, Cigna, and others. We are actively contracting with regional and national payers; exact network status can vary by plan and location.

Kaiser Permanente: panels are often limited or closed to new patients — ask us for current availability. Networks change; your card and employer plan matter, so we always recommend texting to confirm before your visit.

Out of network
If we’re not in network with your plan, we’re still happy to see you when it’s clinically appropriate. We can provide a superbill (itemized receipt) for you to submit to your insurer for any out-of-network benefits you may have.
Self-pay visits
Prefer to pay out of pocket for standard medical visits? Contact our clinic — we’ll explain self-pay visit options and fees for your situation. This is separate from optional self-pay enhanced migraine programs, which use their own agreement and pricing.

Important billing note: Sora's enhanced migraine programs are optional self-pay services. They are not insurance, are not a substitute for health insurance, and may not be covered by insurance. Standard medical visits and covered medical services are billed separately according to the patient's insurance coverage and applicable payer rules. Program fees do not include insurance cost-sharing, prescription medications, procedures, lab testing, imaging, facility fees, or services billed by outside entities unless expressly stated in writing. For Medicare, Medicaid, and commercial insurance patients, Sora follows applicable payer rules and will not charge separately for services that are required to be billed to insurance. Enrollment in an enhanced migraine program is optional and is not required to receive standard medical care at Sora.

Before your visit: bring your insurance card, a current medication list, and any prior imaging or neurology records if you have them. We handle prior authorizations for headache treatments when your plan requires them. Some visits are best in clinic; many follow-ups can be done via secure video when appropriate for your state and clinical situation.
